In one month in 2016, Central Intake & Assessment specialists at the Homeless Hotline …

  • Took 1,233 calls for assistance
  • Problem-solved and provided referrals for 175 households, thereby helping them to avoid entering the homeless system altogether
  • Helped to place 77 households in area shelters
  • Helped to place 10 households in permanent housing

The Homeless Management Information System (HMIS)

The Stark County Homeless Management Information System (HMIS) is a data collection system utilizing a single point of entry (the Homeless Hotline, 330-452-4363) for clients in need of emergency shelter or other homeless services. The HMIS assigns each case a number, which can be used to access various services at different locations throughout Stark County. Data from the HMIS can be used to track who is being served by the programs, who is seeking assistance, trends in the population seeking assistance and the types of assistance provided.

Each Continuum of Care is required to operate an HMIS by the Department of Housing and Urban Development (HUD) for reporting demographic and outcomes data for all local, state, and federal grants. All agencies receiving funding from the Continuum of Care grant must participate in HMIS.  More detailed information about the HMIS requirement can be found at www.hud.gov/offices/cpd/homeless/hmis/. System Performance Targets

State and federal funding sources gauge each Continuum of Care’s progress toward ending homelessness using a set of established performance measures. In accordance with their standards, HCCSC has set FFY2017 Performance Targets for these measures for the different project types in its system. Historical Point-in-Time (PIT) Count Data

Each year, HUD requires communities receiving CoC grants to conduct a Point-in-Time (PIT) Count in January. The purpose of the count is to get a snapshot of the number of homeless individuals and families in Stark County.
